From 14 to 29 May 2012 at the Center of practical skills of KazNMU after S.D.Asfendiyarov first passed the Clinical skills assessment (492 interns) in accordance with an innovative technique on 12 standardized patients and with the involvement of external examiners. The exam was accompanied by video and audio recording and broadcasting.
The clinical skills examination on specially trained actors to play a role in patients with specific diseases, allow evaluating the various readiness of present-day physician. So at 12 stations, there were 12 standardized patients with 12 different diagnoses, such as community-acquired pneumonia, closed head injury, acute appendicitis, etc. Interns had to conduct a survey to collect complaints, history of life and disease, and conduct a physical examination, taking into account the supposed disease. At the end of the encounter with individual patient, interns asked to fill the sheet of the primary examination, which later did a review by independent experts for the scoring in the relevant items of the main valuation list..
In the role of patients were professional actors from local theaters, as well as students of the Academy of Arts. Each actor had a scenario according to which they were previously trained (so-called standardization), which excluded the wrong scenario development of the disease. Also everyone of the standardized patient had a check-list, according to which they evaluated the communicative skills of the modern physician and his professionalism.
Independent examiners were first and highest category clinicians of the central hospitals of the Almaty city. For examiners has been allocated a separate room, where were the 12 monitors and 12 headphones through which they can remotely evaluate the interns. They were not «in contact» with interns, which in turn eliminates the examiner’s subjective attitude to the examinee. Like the preliminary standardization of actors there has been conducted examiners calibration.
Conducting OSKE with standardized patients and independent evaluation gave positive feedback from both the medical interns, and by the examiners that were identified in the survey on both sides.
